Medical Equipment and Digital Systems for a New Hospital: What to Plan Before the Walls Go Up
Equipment and digital systems shape the building, the budget and day-to-day safety. This guide explains how to plan them during design — using needs assessment, lifecycle costing and maintenance planning — and how to build an HIS that can connect to national systems.

- Equipment decisions drive room sizes, structure, shielding, power, water and cooling, so plan them during design.
- Use a structured needs assessment and lifecycle costing rather than a supplier's catalogue.
- Local service support, spares and maintenance capacity matter as much as specification.
- Choose an HIS/EMR that can integrate with national or emirate-level systems where they exist.
- Treat IT infrastructure, cybersecurity and downtime procedures as clinical-safety issues.
In many hospital projects, equipment and IT are treated as fit-out items bought near the end. That is a costly mistake. An MRI scanner dictates structure and shielding; a sterilisation department dictates water, drainage and steam; an EMR dictates network design, power and the way every clinician works.
This guide sets out how to plan both during design, so the building, budget and systems fit together on opening day.
Plan equipment with the building
For each room, record the equipment it will contain and the services that equipment needs — power and backup tier, data points, water and drainage, medical gases, floor loading, ceiling supports, heat output and shielding. Room data sheets are the standard tool for this. Changing a major equipment decision after structural design can mean redesign; changing it after construction can mean demolition.
Needs assessment and lifecycle thinking
WHO’s medical device technical series provides a neutral framework, starting with a needs assessment for medical devices and continuing through procurement, inventory and maintenance. The core questions are: what services will the hospital provide, what volume is expected, what equipment is genuinely needed for that volume, and can it be operated and maintained locally?
| Equipment group | Building implications | Lifecycle questions |
|---|---|---|
| Major imaging (CT, MRI) | Shielding, floor loading, cooling, delivery route | Service contract, uptime guarantees, replacement access |
| Theatres and critical care | Pendants, specialised ventilation, backup power | Consumables, calibration, user training |
| Sterile services | Water quality, steam, drainage, clean/dirty separation | Validation, spare parts, maintenance staff |
| Laboratory | Benching, ventilation, water, waste | Reagent supply, quality control, LIS integration |
| Dialysis | Water treatment plant and loop | Water testing, technician support |

Maintenance and service support
Equipment is only useful if it works. WHO’s guidance on medical equipment maintenance programmes and inventory management stresses inspection, preventive and corrective maintenance, and a reliable inventory as the basis for budgeting and planning. Before buying, confirm whether the supplier has trained engineers and spare parts in-country, what response times are guaranteed, and what the hospital’s own biomedical team will handle. Be cautious with donated equipment that lacks manuals, spares or local support.
Hospertz tipMake the equipment inventory and maintenance schedule a commissioning deliverable. Every item should arrive with its asset number, manuals, service contract details and first preventive-maintenance date already in the system.

The digital core: HIS, EMR, PACS and LIS
A new hospital is the best chance to implement a single, well-integrated digital core — hospital information system and electronic medical record, imaging archive (PACS), laboratory system (LIS), pharmacy and billing — rather than stitching systems together later. WHO’s Global strategy on digital health 2020–2027 encourages countries to build integrated, interoperable systems, and national requirements increasingly reflect that:
- Kenya: the Digital Health Act 2023 established a Digital Health Agency to create an integrated health information system (EY).
- UAE: emirates operate health information exchanges, reported as Malaffi in Abu Dhabi and NABIDH in Dubai (secondary source) — confirm current connection requirements with the regulator.
Ask vendors to demonstrate working integrations in your target market, not just standards on paper.

Infrastructure, cybersecurity and downtime
Digital systems need the same engineering care as clinical ones: secure server or data rooms on backup power with cooling, resilient network design with redundancy for critical areas, wireless coverage for mobile devices, and clear separation of medical device networks. Cybersecurity should be specified in contracts — access control, patching, backups, incident response — and every department needs a tested downtime procedure so care continues when systems fail.
Plan equipment and digital systems with the building and they become assets on opening day rather than problems to fix afterwards. Frequently asked questions
When should medical equipment be planned for a new hospital?
During design. Major equipment affects structure, room sizes, shielding, power, water and cooling, so it should be specified alongside the architectural and engineering design, with room data sheets recording each requirement.
What is a medical equipment needs assessment?
It is a structured process — described in WHO's medical device technical series — that links the services a facility will deliver and the expected volume to the equipment genuinely required, taking into account operation and maintenance capacity.
How do you plan maintenance for hospital equipment?
Build an accurate inventory, define preventive and corrective maintenance schedules, confirm local service support and spare parts, set response times in contracts, and decide what the in-house biomedical team will cover.
Which digital systems does a new hospital need?
Typically a hospital information system and electronic medical record, an imaging archive (PACS), a laboratory information system, pharmacy and billing systems, all integrated, plus the network, data rooms and security to support them.
Does a hospital EMR need to connect to national systems?
Increasingly, yes. Kenya's Digital Health Act created an agency to build an integrated national system, and UAE emirates operate health information exchanges. Confirm current requirements with the relevant regulator for each project.
