Hospital Project Risks: Practical Ways to Reduce Delays, Cost Overruns and Opening-Day Gaps
Hospital projects rarely fail for one dramatic reason. They fail through a chain of small, predictable risks — a vague brief, a late approval, an uncontrolled change, a missing spare part, an unfilled post. This guide sets out a practical risk register and the controls that break the chain.

- Most hospital project problems trace back to decisions that were deferred or never formally closed.
- Map every licence, inspection and accreditation step before design freeze.
- Uncontrolled design change is one of the biggest drivers of delay and overrun; enforce change control.
- Budget explicitly for contingency and for operating losses during ramp-up.
- Operational readiness — people, SOPs and trial runs — is a project deliverable, not an afterthought.
Ask anyone who has delivered a hospital what went wrong, and the answers are remarkably consistent: the brief kept changing, an approval took longer than expected, imported equipment arrived late, the power connection was not ready, key staff had not been recruited, or the hospital opened before its systems were tested.
None of these is unusual, and most are predictable. That is good news — predictable risks can be managed. This guide offers a practical register and the controls that tend to work.
Where hospital project risk comes from
Risk accumulates at the boundaries between stages: from feasibility to brief, brief to design, design to construction, construction to commissioning, and commissioning to operation. Each handover is a chance for an assumption to go unchecked. The seven-stage framework in our guide to planning a hospital from feasibility to opening is built around closing those gaps.
A practical hospital project risk register
| Risk | Early warning sign | Practical control |
|---|---|---|
| Weak or changing clinical brief | Departments disputed late in design | Signed-off clinical services plan; clinical leads engaged early |
| Approvals delay | Regulatory requirements “to be confirmed” | Approvals map with owners and dates before design freeze |
| Design change | Rising number of change requests after tender | Formal design freeze and change-control board |
| Procurement and import delays | Long-lead items not ordered by design stage | Long-lead equipment schedule; early orders for major items |
| Utilities not ready | No confirmed grid, water or sewer connection date | Early utility applications; backup systems in scope |
| Cost overrun | Contingency drawn down early | Realistic contingency, cost reporting by package |
| Workforce gaps | Senior posts unfilled six months before opening | Recruitment plan tied to the construction programme |
| Slow ramp-up | Payer contracts or empanelment not secured | Ramp-up working capital; payer applications before opening |
Illustrative register: adapt risks, warnings and owners to each project.

Approvals: map them before you design
Approval routes are local and sequential, and each one can stop the project. Examples of what must be planned for:
- Kenya: facility registration and annual licensing by KMPDC follow inspection, under rules published in 2022 (Kenya Law).
- South Africa: OHSC inspections against norms and standards, with compliance certificates and renewal applications due within six months before expiry (OHSC).
- Saudi Arabia: CBAHI accreditation is mandatory and has been linked to hospital licence renewal (PMC review).
- Dubai: the DHA publishes a detailed facility licensing manual that sets out the stages.
Confirm current requirements for every project; they change.
Hospertz tipGive every approval in the map a named owner and a “latest safe date” — the last date it can be secured without delaying opening. Review those dates at every project meeting, not just when something slips.
Change control and contingency
Change is inevitable; uncontrolled change is optional. A change-control board — owner, clinical lead, designer and cost manager — should approve any change after design freeze, recording the clinical reason, cost and programme impact. Pair this with a realistic construction contingency and a separate operating contingency for the ramp-up period, when costs run ahead of revenue. Recommendation: keep these two contingencies separate so that construction overruns do not quietly consume the money needed to open safely.

Utilities and resilience
Late or inadequate utility connections are a frequent and underestimated cause of delay. Apply for grid, water and sewer connections early, and include backup power and water in the base scope. In South Africa, load shedding showed how quickly inadequate backup becomes a clinical risk — the Minister of Health said in 2022 that generators had not adequately met demand in some hospitals (SAnews). Operational readiness: the last mile
A building can be complete and still not be ready. Operational readiness means senior staff recruited early enough to write and test SOPs; equipment commissioned, inventoried and on maintenance schedules (WHO guidance); IT systems tested with downtime procedures; payer contracts or empanelment in place; and rehearsed patient journeys before the first real admission. Treat readiness as a project workstream with its own plan, owner and gate review.
Hospital projects that manage risk well are not lucky; they are disciplined. They close decisions on time, map approvals early, control change, protect contingency and treat opening as a project of its own. Frequently asked questions
Why do hospital projects run over budget?
Common causes include an incomplete clinical brief, design changes after tender, late approvals, delayed imported equipment, unprepared utility connections and underestimated ramp-up costs. Most are predictable and can be controlled.
How can hospital construction delays be reduced?
Close each planning stage formally, map approvals with owners and dates before design freeze, order long-lead equipment early, apply for utility connections early, and enforce change control after design freeze.
How much contingency should a hospital project have?
It depends on project maturity and complexity. Rather than a fixed rule, set contingency based on a risk assessment, and keep a separate operating contingency for the ramp-up period after opening.
What is operational readiness in a hospital project?
It is the work that makes a finished building ready for patients: recruitment, SOPs, training, equipment commissioning and maintenance set-up, IT testing, payer contracts and rehearsed patient journeys.
Which approvals are most likely to delay a new hospital?
Facility licensing and pre-opening inspections are common bottlenecks — for example KMPDC inspection in Kenya, OHSC compliance in South Africa, CBAHI accreditation in Saudi Arabia and DHA licensing in Dubai. Map them early and confirm current requirements.
