Hospital Development in South Africa: Infrastructure Norms, Partnerships and the NHI Transition
South Africa combines a large public system, a concentrated private sector and a national health insurance reform that is being phased in amid legal challenge. This guide explains the regulatory and infrastructure framework a new hospital must work within — and how to plan when the payer landscape is still moving.

- Most South Africans use the public system; medical scheme membership was reported at about 15.7% in 2023.
- The NHI Act was signed in May 2024 and is being phased in; some provisions have faced legal challenge, so plan using scenarios.
- Every health establishment must meet norms and standards monitored and enforced by the OHSC.
- IUSS health facility guides set out infrastructure norms for public facilities and are a valuable reference for private projects.
- Energy supply risk has made backup power and on-site generation a core design and budget item.
South Africa has some of the continent’s most sophisticated hospitals and one of its most demanding planning environments. A hospital sponsor has to work within a detailed regulatory framework, a two-tier financing system that is under reform, and infrastructure risks — especially electricity — that directly affect operating costs.
None of that makes new development unattractive. It makes careful planning essential. This guide sets out the framework and the decisions that follow from it.
A two-tier system — and who uses it
Stats SA’s 2023 General Household Survey, as reported by BusinessTech, put medical scheme membership at about 15.7% of the population, with most households using public facilities as their first point of care (BusinessTech/Stats SA). Coverage is far higher in some metros and provinces than others.
For planners, that means private hospital demand is concentrated geographically and closely tied to employment and scheme membership, while public demand is broad and growing. Where a facility sits — and who it is designed to serve — matters more than in most markets.
The NHI Act: plan with scenarios
President Ramaphosa signed the National Health Insurance Bill into law on 15 May 2024, describing implementation as phased, with milestones in each phase (The Presidency). Under the model described by the National Department of Health, a central fund would purchase services from both public and private providers, with the Act phased in according to available resources.
Implementation has been contested. In 2026, reporting indicated that proclamation of certain provisions was postponed pending Constitutional Court proceedings (Financial Afrik). Check the current legal status before relying on any timeline.
Recommendation: build at least three scenarios into the business case — current medical-scheme-led demand, partial NHI contracting, and broader NHI purchasing — and design a facility that remains viable across them.
In South Africa, the question is not whether a hospital will face financing change, but whether its business case and building can absorb it.
Quality regulation: OHSC norms and standards
The Office of Health Standards Compliance was created under the National Health Act to monitor and enforce compliance by health establishments with norms and standards prescribed by the Minister. The norms and standards regulations applicable to different categories of health establishments were promulgated in February 2018 and took effect in February 2019.
OHSC inspections lead to compliance outcomes, and its published guidance describes compliance certificates and the need to apply for renewal within six months before expiry (OHSC Bulletin). The regulations include facility requirements — for example, establishments must ensure engineering services are in place (reg. 15). That makes building design part of compliance, not separate from it.

Infrastructure norms: the IUSS guides
The Infrastructure Unit Support Systems (IUSS) programme produced health facility guides that the Minister published as health infrastructure norms and standards guidelines in June 2014. The guides describe themselves as voluntary standards, with deviations to be motivated during the Infrastructure Delivery Management System gateway approval process for public projects (National Department of Health).
For private developers, the IUSS guides are still useful: they encode local experience on space standards, department planning and engineering, and they reflect how public-sector partners — and potentially future NHI contracting — will judge facilities.

Power: design for supply interruptions
Load shedding exposed how dependent hospitals are on backup systems. In September 2022, the Minister of Health said generators had not adequately met demand during load shedding and that some hospitals had been forced to switch off critical areas; 37 hospitals were then excluded from the load-shedding schedule (SAnews). The Minister’s statement also highlighted unbudgeted increases in diesel and maintenance costs (Department of Health).
Supply has improved at times since then, but the lesson stands: backup power must be sized for long outages, fuel logistics must be planned, and on-site generation such as solar with battery storage can cut both risk and operating cost. See designing resilient hospitals.

Hospertz tipWrite the energy strategy into the design brief: classify loads, set target autonomy hours for generators, specify fuel storage and resupply, and decide early whether to add solar and batteries. Retrofitting energy resilience into a finished hospital costs far more than designing it in.
Partnerships and service delivery
Many of the strongest South African projects combine public need with private delivery capability — through contracted services, shared infrastructure, training partnerships or public-private partnerships. The President’s NHI remarks explicitly emphasised more effective public–private collaboration (The Presidency). Whatever the structure, agree risk allocation, service obligations and performance measures in writing, and test them against the NHI scenarios above.
South Africa rewards hospital projects that are compliant by design, resilient by default and financially robust under more than one future. [Confirm Hospertz service or capability]: add any Hospertz support for South African projects here before publishing.

Frequently asked questions
What is the status of the NHI Act in South Africa?
The NHI Act was signed into law on 15 May 2024 and is to be phased in. In 2026, reporting indicated that proclamation of certain provisions was postponed pending Constitutional Court proceedings. Check the current status with the National Department of Health before planning around specific dates.
Who regulates quality in South African hospitals?
The Office of Health Standards Compliance monitors and enforces compliance by health establishments with norms and standards prescribed by the Minister of Health. The applicable regulations took effect in February 2019.
What are the IUSS health facility guides?
They are health infrastructure norms and standards guidelines developed through the Infrastructure Unit Support Systems programme and published in 2014. They are voluntary standards, with deviations motivated through the public infrastructure gateway process, and are widely used as a planning reference.
How many South Africans have medical aid?
Stats SA's 2023 General Household Survey, as reported by BusinessTech, put medical scheme membership at about 15.7% of the population, with large differences between provinces and metros.
How should a new hospital in South Africa plan for load shedding?
Classify electrical loads by clinical priority, size generators for extended outages, plan fuel storage and resupply, and consider solar with battery storage to reduce diesel dependence. Test changeover regularly.
